Terms and Conditions

1. Pfizer Inc. will make a donation for each promise (per unique e-mail address) submitted. We will donate $1—up to $100,000 total—to the following organizations involved in helping people with fibromyalgia:



• The National Fibromyalgia Association
• The American Chronic Pain Association
• The American Pain Foundation
• The National Pain Foundation
2. Only one donation will be accepted for each unique e-mail address.
3. E-mail addresses will not be used for marketing purposes. Please see Pfizer Privacy Policy.
4. Donation Program will run through the 2008 calendar year.
5. The total donation amount will be divided evenly among the 4 identified organizations.
6. The donations made by Pfizer through this program cannot be claimed as a tax deduction by you.
7. Pfizer Inc. reserves the right to discontinue the online donation program at any time.
